Referrals
Many insurance plans require a referral from your primary care provider to see a specialist. To avoid having to pay out-of-pocket costs, please carefully read your benefit information to determine your insurance company's policy on referrals and specialty care.

It is your responsibility to request a referral from your primary care physician prior to your visit at our clinic. If the referral has not been issued by the time of your visit, you will need to reschedule your appointment. Please keep in mind that authorizations for specialty care have time and visit limitations. Additional authorizations may be required if your circumstance changes.

Insurance Plans
If we are a contracted with your insurance plan, we will bill them directly. Any remaining balance for non-covered benefits, deductibles, and co-insurance are your responsibility. Payment for any patient responsibility is due upon receipt of your monthly statement.

If we are not a member of your insurance network, we will be happy to file a claim with your plan, but please be advised that your visit will likely be treated as an "out of network" visit, which may result in you being responsible for a paying a larger portion of the charges.

Benefits/Coverage
It is your responsibility to understand your insurance benefits. Please be aware that the filing of insurance claims does not guarantee payment by your carrier. Covered and non covered services, deductibles and various exclusions cannot always be determined until your carrier processes the claim. Therefore, we submit insurance claims with the understanding that you are responsible for any non-covered service, co-payments and/or deductibles. If you are unsure of your benefits it is always a good idea to call your health insurance carrier prior to your visit.

Co-Pays
All co-pays are expected at the time the service is rendered. It is your responsibility to notify the receptionist upon arrival that you have a co-pay. A $20.00 handling fee maybe added to your statement if you do not pay your co-pay at the time of service.

For patients without health insurance, we are happy to extend a 25% cash discount to you for payment at the time of your visit. All fees are due at the time of service.
